Through research, it was been noted that effective transition planning is critical to the success of students with disabilities and exceptionalities.
(2011-2013):
A working group has already been established to promote the sharing of best practices, research data, and learning practices that support the transition of students with disabilities. This project mobilizes knowledge by developing this working group. Specifically, it will: (1) develop resources ; (2) research student outcomes ; (3) strengthen the development of a professional knowledge network ; (4) develop a website ; and (5) provide a workshop .
The resources that will be developed include an implementation of transition checklist to be used by school boards and agencies to allow educators, parents, and students to become more aware of barriers and issues that they need to prepare for when the student transitions into post-secondary. In addition, the self-directed IEP process will be further developed to develop advocacy skills in students. The research component involves sharing data on student outcomes already provided to the Ministry of Training, Colleges and Universities, to secondary schools, and to community agencies in order to develop successful transition programs. The professional knowledge network will allow for the better flow of best practices and program data to inform school board policy and community planning for students with disabilities. The website and workshop are both intended for educators to share information on transition activities and practices.
